Quick Answer
At 20 weeks, your baby is about the size of a papaya (~25cm, ~300g). You’re officially halfway through your pregnancy. This is a landmark week — your baby’s brain is developing specialised sensory areas (taste, smell, hearing, sight, touch), and the anomaly scan window is at its optimal point. Baby has doubled in size in just a few weeks and is now measured head-to-heel instead of head-to-rump. Aadha raasta ho gaya — well done.
Baby Development at Week 20
A huge amount is happening in your baby’s brain and body:
- Brain developing sensory areas — all five senses now have dedicated regions in the brain that are rapidly forming neural connections
- Size jump — from this week, baby is measured from head to heel (not head to rump), which is why the number jumps from ~15cm to ~25cm. Baby hasn’t suddenly doubled — it’s just measured differently
- Weighs ~300g — gaining weight steadily now
- Can hear clearly — conversations, music, your heartbeat, digestive sounds
- Taste buds active — baby can taste sweet vs. bitter in the amniotic fluid
- Regular sleep-wake cycles — baby sleeps about 12-14 hours a day, and is increasingly active during waking periods
- Swallowing and producing meconium — the first stool (dark, sticky substance) is forming in the intestines
Your baby is now about the size of a papaya from head to foot. The vernix and lanugo continue to protect the skin.
Your Body at Week 20
The halfway mark. Your uterus is roughly at the level of your navel.
Common Symptoms
| Symptom | What’s Happening | What Helps |
|---|---|---|
| Clear baby movements | Kicks, rolls, and jabs are becoming more distinct | Enjoy them. Lie down after a meal to feel them better |
| Braxton Hicks contractions | Uterus tightens for 30-60 seconds and releases. “Practice” contractions | Drink water. Change position. If regular (every 10 min) or painful, call doctor |
| Backache | Centre of gravity shifted significantly. Relaxin hormone loosening joints | Good posture. Supportive pillow. Prenatal yoga. Warm (not hot) compress |
| Leg cramps | Especially at night. Weight, circulation, and mineral needs increasing | Stretch before bed. Magnesium-rich foods. Hydrate well |
| Shortness of breath | Growing uterus pushes diaphragm upward | Stand straight. Sleep propped up. It’s normal — baby is getting enough oxygen |
| Swelling (edema) | Feet, ankles, and fingers may swell, especially by evening | Elevate legs. Reduce salt. Drink more water. Remove rings if they’re getting tight |
Sleeping Position
From 20 weeks onward, avoid sleeping flat on your back for extended periods. The weight of the uterus can compress the inferior vena cava (major vein carrying blood back to heart), reducing blood flow to you and baby. Side sleeping — ideally left side — is best. Don’t panic if you wake up on your back; just turn over.
Tests & Screenings Due
Anomaly / TIFFA Scan — Do It This Week
20 weeks is the optimal time for the anomaly scan. This is the most comprehensive ultrasound of the pregnancy.
What the sonographer checks:
- Brain: Ventricles, cerebellum, midline structures
- Face: Cleft lip screening, nose, orbits
- Spine: Every vertebra from cervical to sacral
- Heart: Four-chamber view, outflow tracts, great vessels. Congenital heart defects are the most common birth defects — this check is critical
- Lungs and diaphragm
- Abdomen: Stomach, kidneys, bladder, liver, abdominal wall
- Limbs: Long bones measured. Hands and feet
- Placenta: Location (anterior, posterior, fundal). Low-lying placenta noted — most move up as the uterus grows
- Cervical length: Measured transvaginally if indicated. Short cervix (<25mm) needs follow-up
- Amniotic fluid index (AFI): Normal range checked
- Umbilical cord: Three vessels (2 arteries, 1 vein)
The scan takes 30-45 minutes. If views are incomplete (baby’s position), you may need a follow-up. Ask for the report — read through the findings. Sab “normal” likha hona chahiye.
Blood Tests
- Hemoglobin recheck — your doctor may order this around 20 weeks to catch any developing anemia
- Blood sugar — if you have risk factors (family history of diabetes, PCOS, obesity, age > 30), your doctor may screen for GDM now. Universal GDM screening is at 24-28 weeks
Nutrition This Week
What to Prioritize
- Iron: 60 mg/day — your blood volume is at its highest production phase. Don’t skip the supplement
- Calcium: 1000 mg/day — baby’s bones are hardening rapidly
- Protein: at least 70g/day, increasing to 78g/day (ICMR) — increase from earlier weeks as baby grows
- Water: 3 L/day — edema is counterintuitively improved by drinking more water, not less
- Potassium — helps with cramps and fluid balance. Bananas, coconut water, sweet potatoes
Halfway Celebration Meal Ideas
You’ve earned a treat. But keep it nutritious:
- Paneer tikka with mint chutney (protein + calcium)
- Grilled fish with lemon (protein + omega-3 + vitamin C)
- Fruit chaat with chaat masala (vitamins + fibre + joy)
When to Call Your Doctor
- Any vaginal bleeding — always urgent
- Gush of fluid — premature rupture of membranes
- Regular, painful contractions — before 37 weeks, every 10 minutes or more frequent
- Severe headache with visual changes — could indicate pre-eclampsia
- Severe upper abdominal pain — especially right side under ribs
- Sudden severe swelling — face, hands
- Decreased movements after 22-24 weeks — if baby was active and suddenly stops
What’s Coming Up
- Week 21: Coordinated movements. Bone marrow begins producing blood cells
- Week 24: Viability milestone — baby has a chance of survival if born extremely premature
- Week 28: Third trimester begins. Glucose tolerance test. Anti-D injection for Rh-negative mothers
Aapke Sawaal (Common Questions)
Anomaly scan mein koi problem mile toh?
Zyaadatar anomaly scans normal aati hain — ye reassure karne ke liye hoti hain. Agar koi soft marker ya concern mile, toh doctor aapko detail mein samjhayenge. Soft markers (like echogenic focus, mild pelviectasis) aksar normal variants hote hain. Agar koi major finding ho, toh aapko fetal medicine specialist ke paas refer kiya jayega aur options discuss honge.
20 weeks mein baby kitna hilta hai?
Har baby alag hai. Kuch bahut active hote hain, kuch quieter. 20 weeks mein abhi fixed pattern nahi hota — regular kick counting 28 weeks se start hota hai. Abhi ke liye daily kuch movements feel ho rahi hain toh achha hai. Agar bilkul kuch nahi feel ho raha aur 22 weeks ho gaye hain, toh doctor ko bataayein.
Braxton Hicks aur real contractions mein kya fark hai?
Braxton Hicks irregular hote hain — kabhi aate hain kabhi nahi, timing fixed nahi hoti. Uncomfortable hote hain lekin painful nahi. Position change karne ya paani peene se band ho jaate hain. Real contractions regular hote hain (har 10-15 min), intensity badhti jaati hai, aur position change karne se fark nahi padta. Agar regular pattern mein aa rahe hain before 37 weeks, toh turant doctor ko call karein.
