Your Baby This Week

About the size of a mango

Length: ~14cm

Quick Answer

At 18 weeks, your baby is about the size of a mango (~14cm). Your baby is now covered in lanugo — fine, downy body hair that helps regulate temperature. A sleep-wake cycle is developing, and baby can respond to loud noises. The anomaly scan window opens this week. And the biggest excitement? You might feel your first baby movements any day now. Pehli baar movement feel karna ek alag hi experience hai.

Baby Development at Week 18

Your baby is becoming more active and responsive:

  • Covered in lanugo — fine, downy hair covers the entire body. It holds the vernix (protective coating) in place and helps with temperature regulation. It sheds before birth
  • Sleep-wake cycle developing — baby now has periods of activity and rest. You may notice patterns — active at night, quiet during the day (unfortunately)
  • Responds to loud noises — baby may startle at sudden sounds. The hearing system is functional
  • Yawning and hiccupping — visible on ultrasound. These are practice movements for breathing and swallowing
  • Nervous system maturing — myelin (insulating coating) is forming around the nerves, enabling faster signal transmission
  • Genitals fully visible — on ultrasound, the anomaly scan will clearly document reproductive anatomy

This is an active time — baby is rolling, kicking, and stretching. The uterus still has plenty of room.

Your Body at Week 18

Your bump is clearly visible now. Most people will notice you’re pregnant.

Common Symptoms

SymptomWhat’s HappeningWhat Helps
First movements (quickening)Fluttering, popping, or bubbling sensations in lower belly. Often mistaken for gas at firstLie still after a meal — you’re more likely to notice movements when you’re relaxed
BackacheGrowing belly pulls spine forward. Ligaments looseningGood posture. Supportive shoes. Pregnancy pillow. Gentle stretching
Increased appetiteBaby’s growth is accelerating — your body needs more fuelEat nutrient-dense food. +350 kcal/day. Don’t fill up on empty calories
Leg crampsOften at night. Related to weight, circulation, and possibly calcium/magnesiumStretch calves before bed. Bananas, almonds, milk. Stay hydrated
Swollen feet/anklesFluid retention increases. Standing for long periods makes it worseElevate legs. Don’t stand still for long. Reduce sodium. Drink more water (counterintuitive but works)
Dizziness on standingBlood pressure drops when you stand quickly (orthostatic hypotension)Stand up slowly. Don’t skip meals. Sit down if you feel lightheaded

Quickening — What Does It Feel Like?

First-time mothers often describe it as:

  • Butterflies in the stomach
  • Gas bubbles
  • A gentle popping or tapping
  • Something “swimming” inside

Ye pehle bahut subtle hota hai. If you haven’t felt it yet at 18 weeks — perfectly normal. Most first-time mothers feel it between 18-22 weeks. Second-time mothers often feel it from 16 weeks because they know what to look for.

Tests & Screenings Due

Anomaly Scan / TIFFA Scan (18-22 Weeks)

This is the most important ultrasound of the entire pregnancy. The window opens now, with the optimal time being around 20 weeks.

What it covers:

  • Head: Brain structure, skull shape, face
  • Spine: Entire spine checked for defects (spina bifida)
  • Heart: All four chambers, valves, outflow tracts (detailed cardiac check)
  • Organs: Stomach, kidneys, bladder, diaphragm, intestines
  • Limbs: All four limbs, hands, feet
  • Placenta: Location (important for ruling out placenta praevia)
  • Cervical length: Short cervix can indicate preterm labour risk
  • Amniotic fluid: Too much or too little

This scan typically takes 30-45 minutes. If the baby’s position doesn’t allow complete views, you may need to come back.

Quadruple Marker — Last Window

If you haven’t done the quadruple marker test yet, the window closes at 20 weeks. Discuss with your doctor.

Nutrition This Week

What to Prioritize

  • Protein: 60-70g/day — critical as baby’s muscles develop. Include a protein source at every meal
  • Iron: 60 mg/day — baby is now building its own blood supply. Your iron needs increase
  • DHA/Omega-3 — brain development is rapid. Walnuts, flaxseeds, fatty fish
  • Fibre — constipation is an ongoing battle. Whole wheat roti, oats, fruits with skin, salads
  • Small frequent meals — heartburn is better managed with 5-6 smaller meals than 3 large ones

Snack Ideas

  • Curd with roasted chana and a pinch of chaat masala
  • Fruit with a handful of nuts
  • Paneer tikka or grilled chicken
  • Ragi ladoo with jaggery and coconut
  • Smoothie with banana, milk, and flaxseeds

When to Call Your Doctor

  • Any vaginal bleeding — always urgent after the first trimester
  • Gush of clear fluid — could be premature membrane rupture
  • Regular contractions — tightening that comes and goes at regular intervals before 37 weeks
  • Severe headache with visual changes
  • Fever above 100.4°F (38°C)
  • No fetal movement felt by 22 weeks — if this is your first baby and you haven’t felt anything by 22 weeks, mention it to your doctor
  • Sudden severe swelling — of face, hands, or one-sided leg swelling

What’s Coming Up

  • Week 19: First kicks often felt! Unique fingerprints present. Hiccupping begins
  • Week 20: Halfway point! Anomaly scan (if not done yet). Brain developing sensory areas
  • Week 21: Coordinated movements. Bone marrow producing blood cells

Aapke Sawaal (Common Questions)

Anomaly scan mein kya dekhte hain?

Anomaly scan (TIFFA scan) pregnancy ka sabse detailed ultrasound hai. Isme baby ka head, brain, spine, heart (all 4 chambers), kidneys, stomach, limbs, placenta, cervical length — sab check hota hai. Ye 18-22 weeks mein hota hai, ideal time 20 weeks hai. Typically 30-45 minutes lagta hai. Agar baby ki position se sab nahi dikh paata, toh dubara aana pad sakta hai.

Mujhe abhi tak movement feel nahi ho raha — tension leni chahiye?

Nahi. Pehli pregnancy mein 18-22 weeks tak movement feel hona normal hai. Bahut si women ko 20 weeks tak pata hi nahi chalta ki wo movements hain ya gas. Agar aapki placenta anterior hai (belly ke taraf), toh movements aur late feel hote hain kyunki placenta cushion ki tarah kaam karta hai. 22 weeks tak na ho, toh doctor ko bataayein.

Kya ab left side pe sona zaroori hai?

Left side pe sona best maana jaata hai kyunki isse baby ko blood supply better hoti hai (inferior vena cava pe pressure kam padta hai). Lekin right side bhi safe hai. Bass seedha (flat on back) sona avoid karein — especially 20 weeks ke baad. Agar raat mein karwat badal jaati hai toh tension mat lein, pair ke beech pillow rakhein toh comfortable rehta hai.

Week 18 Checklist

  • Schedule your anomaly/TIFFA scan (18-22 weeks, optimal at 20 weeks)
  • Pay attention to fluttering sensations — could be first baby movements
  • Continue iron 60 mg/day and calcium supplements
  • Eat protein-rich food at every meal
  • Sleep on your left side — better blood flow to baby
  • Stay active — prenatal yoga, walking, swimming
  • Start planning your maternity leave and birth plan discussions
